Catalogs: Fix long tags causing catalog generation to fail on windows. Add the ability to cross-reference authors, i.e. to relist the authors for a book with multiple authors separately. Fixes #1074931 (Catalog Creation fails with version 0.9.5)

The option should be available because (i) the option is off by default and therefore the most novice users (that are least likely to identify the unhandled exception when it occurs) will be unaware of the option and (ii) the option has a larger net value available than not available.

Implement font subsetting. calibre can now subset all embedded fonts (i.e. reduce every embedded font to contain only the data for characters used in the book). This can often greatly reduce the size of the ebook file. Note that currently only fonts with TrueType outlines are supported not PostScript (CFF) outlines
